Diabetes Mellitus in Rodenticide " RH - 787 " Intoxication

Abstract
Six cases of intoxication of RH-787 (N-3-pyridylmethyl-N¢¥-p-nitrophenylurea), a new class of rodenticide were analyzed clinically and the results obtained were as follows.
1. Subjects included were three cases of male and three cases of female ranging from 20 to 67 years of age, and they ingested about 20 to 30 grams of RH-787 for suicidal purpose.
2. Clinical symptoms could be summarized as general weakness, blurred vision, headache, abdominal distress, burning foot and drowsiness. Urinary retention was noted in 4 (66. percent) of 6 cases.
3. All cases examined slowed as abnormal laboratory findings leukocytosi5, fasting glycosuria and ketonuria with elevated blood glucose level (260 to over 400 mg/100 ml), and increased protein (65 to 190 mg%) and glucose amount 0160.4 to 200mg%) in the cerebrospinal fluids.
4. The liver function tests, electrocardiograms, electroencephalograms, and roentgenological examinations of the chests were within normal range.
5. Liver biopsy showed focal necrosis and fatty degeneration. Kidney biopsy showed degenerative changes in tubular epithelium.
6. Continual injections of NPH insulin, 10 to 88 units daily was done in long survival cases for control of hyperglycemia.
Responses could not proved to administration of tolbutamide, large doses of nicotinamide and ¥á-adrenergic blocker.
7. Durations of survival were 4 day to about 7 months. Overall mortality was 86.7 percent.
